Option 1- INSURANCE:

Please, be advised, that Dr. Kelly is an IN- NETWORK provider for BLUE SHIELD and AETNA only.

Fill out the insurance verification form below PRIOR to your first visit. We can verify your coverage, benefits, deductibles, co-pay, co-insurance and all relative information. Bring your ID and Insurance Card on your first visit.

We accept Cigna, United Health Care and several PPO plans, after your out-of-network deductibles have been met.

Option 2- CASH:

Fees are paid at the time of the visit, unless option 4 is in place. Please discuss at your free consultation.

Option 3- PRE-PAID SIGNATURE PACKAGE

If you are considering this option, please discuss it at your first visit. Packages are available for non-fertility patients as well.

Option 4- WEEKLY/MONTHLY CASH PAYMENT

For those transient, but active patients, who qualify, we will extend the credit through this plan; however, should you become inactive or discontinue care your entire unpaid balance will be due immediately.
